Pioneer Medical Center (PMC) provides care for patients who are discharged from the hospital but are not quite ready to go home. PMC has a newly remodeled unit designed especially to help these patients transition from the hospital back to their homes.
PMC's transitional care patients are cared for by hospital acute care trained nursing staff with a physician available 24-7 for any care needs that may arise. PMC therapy services include: physical therapy, occupational therapy, speech therapy. We also provide registered dietitian services, and social services.
Our transitional care team meets daily to coordinate care and make certain we are meeting all the needs of our patients.
